Niyat for the five daily prayers/namaz are below in arabic, with transliteration and translation. Use find feature or scroll down manually to what you are looking for.
Nowadays the position on salah intention/niyat is that it does not need to be said verbally but in the heart. However, one should still use some aspects of the niyat below to distinguish the salah you plan to perform.
Below is how niyats were made for those taught in the late 20th century. It is a little tough to learn the niyat below however, most parts of the niyat are always the same.
Heart to Heart (H2H): From what I was able to find, the view on intention is that it is made naturally in the heart so it is not something that needs to be said nor is there record of it being said. However, intention to pray should be more special than intention to do other common things in my view. Prayer is offered to Allah (SWT) so it should be a special intention. Also, making an intention in the heart is difficult unless you have high iman/faith and you pray regularly. Typically your intention will be made verbally or in your mind. Nevertheless, how you do it up to you and I encourage you to do your own research.

Nawai tuan usallia lillahi ta’ala
I turn to Allah the most high with the intention for

Niyat Add Ons
If one does not pray a salah at the appropriate time, the salah is considered qadza/late. One should add the word “faaetate” to their niyat after the name of the salah to makeup the prayer which means missed. The rest of the niyat stays the same.
If one is following praying behind an Imam, one should add the phrase “eqtadaitu behadzal imami” to their niyat before saying “muthawaj-jihan” which means “I follow this imam”. The rest of the niyat remains the same.
If one is leading the prayer as an Imam, one should add the phrase “ana imamul leman hhadwara wa yahhdwiru” to their niyat before saying “muthawaj-jihan” which means “As the Imam I am leading those who are present and those who will attend.” The rest of the niyat remains the same.
